Information
Students needing to change personal information may do so by filling
out a Change of Information form. The form can be obtained from
the Office of Student Affairs. The form can also be downloaded here.
Requests to change your name must also be accompanied by one of
the following: driver’s license indicating new name, social
security card indicating new name.
You may also change your address, telephone number, email address
and emergency contact information through Campus Connect https://cc.ivytech.edu.
After logging into Campus Connect, go to the Student Services Tab,
and click on the 'Students click here to' link.
