The College may waive the general fees for enrollment up to fifteen (15) credit hours per semester of Ivy Tech courses or an equivalent number of non-credit courses, taken by dependent children and/or spouse of a full-time employee (cost of non-credit courses not to exceed credit course fees for fifteen credit hours).

THIS APPROVAL FORM MUST BE PRESENTED AT THE TIME OF REGISTRATION
