Sellersburg campus will provide test-proctoring service by
appointment for Sellersburg students taking distance courses.

Steps
to Arrange for a Proctor:
- Student
alerts instructor that the Testing Center will proctor the exams.
- Instructor
completes the Ivy Tech Community College-Sellersburg Test Proctoring
Form. This form may be obtained at http://www.ivytech.edu/sellersburg/Distance/r13
proctor form word.doc
- Instructor
attaches proctoring form to the test and sends to the Sellersburg
campus. Mail to: DE Proctor Ivy Tech Community College

- Students
must make an appointment to take the test. The preferred
method is by email at: r13deproctor@lists.ivytech.edu
or by phone at (812) 246.3301, extension 4156 or 4231, toll
free at 1.800.321.9021. When making an appointment, please give
yur first and last name, course name, instuctor, exam number,
date and time of appointment and phone number if calling.
- Students
must present the proctor with a valid photo ID.
- Tests are
distributed up to one hour before closing time. All exams must
be completed within 10 minutes of closing time.
Students
should be aware that Ivy Tech Community College-Sellersburg will
comply with the following test proctoring procedures:

Proctor
information is transferred between Ivy Tech personnel from
the instructor to the proctor and cannot be carried by students.

| b. |
Proctors
will not grade exams or be responsible to distribute homework
or handouts not directly related to the exam. |
| c. |
Tests
must be accompanied by the statewide proctoring form. |
| d. |
Sufficient
time must be allowed to send, administer, and return exams.
The proctor must receive the exam 48 hours prior to the student
deadline for test completion. Exams must be returned to the
originating site within 48 hours of completion. |
| e. |
The originating
site must inform the receive site of special student accommodations. |
| f. |
Students
may not take personal items into the testing room, except
those testing tools noted by the instructor on the proctoring
form. Personal items will be secured in a locker. Examples
of items not allowed in the testing room: briefcases, backpacks,
notebooks, food, drink. Cellular phones, pagers, and watch
alarms must be disabled if taken into the testing room. |
| g. |
Proctor
will administer exam according to the proctoring form completed
by the instructor, regardless of any verbal instructions given
by students. Any irregularities will be logged and reported
to the instructor on the proctoring form. |
| h. |
If the
student chooses another qualified proctor (i.e. faculty or
student services staff at another university, community college,
or K-12 setting; education officers or other qualified staff
at a military base; human resource officers and their staff
at business and industry sites, and librarians), the student
must provide the instructor with the following information
about their proctor: name; address, phone number, fax number
and email address. |

No proctoring
fees for Ivy Tech exams. A $45 fee is charged for all other
DE exams. |
