| Facilities & Grounds Use Request Form for Community Groups/Organizations
Primary consideration for use of College facilities will be given to all College-related activities. Secondary use of College facilities will be given to outside organizations based on the following criteria.
The person submitting this form agrees on behalf of the organization to the following terms and conditions.
- This organization hereby agrees to indemnify, defend and hold harmless the College from any and all claims for damages or injuries to persons or property arising from, or related to the organization’s use of the facilities/grounds. I hereby certify that I have read and fully understand the guidelines governing the use of the facilities and agree to abide by and strictly enforce these guidelines.
- A certificate of insurance MUST be on file with the College prior to this request being finalized.
- College facilities shall not be used by private citizens or organizations for political purposes.
- If Ivy Tech is an event co-sponsor it must approve all advertising through its Regional Relations office. The use of the Ivy Tech logo is not permitted without consent and the College name should only be used to indicate location.
- Use of facilities shall not impede nor interfere with College operations.
- Applicants shall be responsible for the activities and attendees of the event.
- Damage to property, theft or loss of equipment and supplies arising from the occupancy of any portion of the facility shall be charged to the applicant/organization. All equipment used shall be returned to its original state.
- Use of alcohol and other intoxicants is prohibited. Smoking within any College building is prohibited. Smoking shall be permitted in personal vehicles only.
- The activities of the organization shall be restricted to the areas indicated on the request.
- If the College is closed for any emergency, all scheduled use of campus facilities shall be automatically cancelled. College closings are broadcast through the local media.
- Facilities are not available for activities that compete with College sponsored programs.
- Approval/disapproval of facilities requests will include related regional directors and any negotiation of coordination fees assessed shall be the responsibility of the Executive Director of Workforce and Economic Development to complete and document. The WED director will use accepted procedures for pricing, contracting, invoicing, and coordination of services charged.
- Individual applicants or organizations must submit their request for facility use no later than 14 days prior to the scheduled date of the event.
- Emergency contact procedure information will be provided to the organizational representative for use and reference during activities held at Ivy Tech locations.
- If facilities/grounds are requested by any user at times when normal custodial or security services are not provided or if the activity in any way requires such services above and beyond those normally available, the College may charge those costs to the user.
