High School Transcript Request

All degree seeking students must have a high school transcript or GED scores on file before starting their second term at Ivy Tech. A request for transcript should be filled out at your COMPASS testing session. Students that have not yet completed high school or a GED should discuss their situation with their academic advisor.

To request a copy of your transcript from your high school, please print, complete, and mail the form from one of the links below to your high school or testing center where you received your GED.
