Bio/Demographic Information

Students needing to change personal information may do so by filling out a Change of Information form. The form can be obtained from the Office of Student Affairs. You may also request a form by calling the Student Affairs Office at 765-966-2656 ext 1202 or 1-800-659-4562 ext. 1202. The form can also be downloaded here.

Requests to change your name must also be accompanied by one of the following: driver’s license or social security card indicating new name.

You may also change your address, telephone number, email address and emergency contact information via Campus Connect.
