Bio/Demographic Information
The most convenient way to change your address, telephone number, personal email address or emergency contact information is by selecting the Student Services Tab and navigating to the Personal Information area via Campus Connect.
Students may also do so by filling out a Change of Information form. The form can be obtained from the Office of Student Affairs. You may also request a form by calling the Student Affairs Office at 765-966-2656 ext 1202 or 1-800-659-4562 ext. 1202. The form can also be downloaded here.
Note: Requests to change your name must also be accompanied by supporting documentation such as a driver’s license or social security card. You will need to complete this action on campus.
