Library Card Application

Notice to Students: It is not necessary to fill out this application if you are going to be on campus for your classes, and can come to the library. Student information is now automatically entered into the library system each semester, so in most cases, all we need to do is verify your identity with photo ID and we can issue a library card to you in under a minute. Instructors and off-site students should continue to use this form.

You may use this form to apply for a library card for the Anderson, Marion, or Muncie Ivy Tech campuses. ***You must be a registered student at Ivy Tech to receive a student library card.****

You may pick up your card at the campus where you attend or work by showing a photo ID within one week of submitting this form. (Note: If your card is not retrieved within one month, it will be deleted from the system.)
