Full-Time Employee Handbook
Health Care Programs

Eligibility

If you are a benefits-eligible employee, you are eligible to enroll for health insurance. You may also enroll your eligible dependents for coverage. All eligible employees can enroll for employee, employee plus spouse, employee plus child(ren), or family coverage within 31 days of their hire date or during the annual open enrollment period.

Non-disabled dependent children are eligible for coverage to the end of the month in which the child reaches age 19 (for non-students); or to the end of the month in which the child reaches age 24 (for full-time students), per IRS Code 152 definition.

Employees also have 31 days from a qualifying event to make changes to their coverage. See your Human Resources Administrator for more information.

Expense

The College pays a majority of the premium expense for health coverage for you and any eligible dependents you elect to enroll. You pay the remaining portion of the premium expense by payroll deduction. Premiums for the Select Plan are greater than premiums for the Standard Plan and if you wish to enroll in the Select Plan you will bear the full cost of the difference in premium.  In addition, the Tax Saver Program lets you make these payroll deductions on a before-tax basis. For more information on Tax Saver, see the chapter entitled Other Benefits.

Standard Plan

The Standard Plan covers most routine services and prescription drugs without requiring you to meet any deductible.  For some in-network services you are required to meet an annual deductible before the Plan’s 90% coverage is applicable; out-of-pocket costs are low and most preventive care services are covered.

Although the Standard Plan does provide coverage if you use out-of-network providers, you must first meet a $10,000 single, or $30,000 family, annual deductible before coverage is applicable.  Therefore, if you want the flexibility to utilize out-of-network providers on a regular basis, the Select Plan may be the best plan for you.

Select Plan

The Select Plan covers many routine services and prescription drugs without requiring you to meet any deductible.  For other in-network services you are required to meet an annual deductible before the Plan’s 90% coverage is applicable.

The out-of-network deductible is much lower than the corresponding deductible for the Standard Plan.  This feature offers you more flexibility in choosing providers.  In the Select Plan, if you choose a provider in the PPO preferred network, your deductible and co-payment will be less. If you receive services from a provider not in the preferred network, your deductible and co-payment will be greater. You have the flexibility to use in-network or out-of-network providers at your discretion, keeping in mind the different benefit levels. Under this plan, your in-network and out-of-network deductibles accumulate separately depending upon the provider's designation as either in- or out-of-network.

Please contact your regional Human Resources Administrator or visit the College's benefits website at www.gregoryappel.com/ivytech.edu to determine which health plan is the best for you, for plan details, and premium rates.

For complete details of health plans, services covered and exclusions, please refer to your health plan booklet.
