Transcript Request

If you are planning to continue your education at another college or university after attending Ivy Tech, you will need a copy of your Ivy Tech transcript to complete your application process. Also, some employers will want to see a copy of your transcript, as a condition of your application. To request an official Ivy Tech transcript, you must make your request in writing. For your convenience and ours, please use the Ivy Tech Transcript Request form.

The Transcript Request form is available online in PDF format. In order to view and print this form, you will need the Adobe Acrobat Reader. To obtain the Transcript Request form, please click on the link below. Once you've printed this form, please fill it out in pen, making sure to include all requested information. Remember that the Transcript Request form cannot be processed without a signature. Once the form is completed, please return the form to the Registrar's Office, either in person, or by mail to:
