Audit Request Form

An Audit Request form is used to request permission to attend and participate in an instructor's class without getting a grade or credit. You must request to audit a class by the end of the first week of the term, and the instructor and program chairperson must approve the audit. All prerequisites must be met or the appropriate waiver(s) signed. Once an audit request is made, it can not be changed to a grade status later during the term.
