How do I apply for a job with Ivy Tech?

I’ve moved recently. How do I change my address?

You can change your address and phone number by utilizing the option on Banner Employee Self-Service. Please refer to your Banner Self-Service Manual for directions.


The old pay stubs we received always showed our vacation and sick time accruals. I can’t find this information on the pay stubs we print from Banner.

This information is available to you by using Banner Employee Self-Service. Go to the Contents section and it is listed under “Leave Information.”


I just recently got divorced (or married) and have a new last name. How can I get that information changed so that it is reflected on my employee record and other College information?

You will need to present Human Resources a Social Security card reflecting your name change. Only at that time are we allowed to make a name change.


How can I find out who’s listed as my life insurance and retirement beneficiary? 

Contact Rhonda Burch at extension 4186, and she will assist you with that information. She can also provide you with the necessary forms in the event that you would want to make a change.


What type of situation constitutes the need for an Incident Report?

An Incident Report (which you can find in the “Forms” section) should be completed any time there is an injury or illness that a First Responder is called to assist with. Other situations would include: auto mishap in parking lot; theft of College or personal items; slip and falls; suspicious activity; bomb threat; assault; inappropriate conduct and verbal altercations.
