The Department of Education randomly selects a portion of the student population for a process called verification. This process gives Ivy Tech the authority to request copies of certain financial aid documents from you and your parents or spouse.

The links below are for financial aid documents and forms you may be asked to submit to complete your application for assistance.

You may also need to provide Ivy Tech with other documents from home, such as copies of your federal tax return. All tax documents must be signed by the taxpayer. Instead of a return signed by the filer, we may accept a paper return on which the tax preparer has stamped, typed, signed, or printed her name and her Social Security Number (SSN), Employer Identification Number (EIN), or Preparer Tax Identification Number (PTIN).

Click on the desired form below, print it out and fill in the necessary data, including the appropriate signatures. Once you complete the form(s), please mail, fax or hand-deliver them to the Financial Aid Office at your campus.
