Disability Support Services
REQUESTING ACCOMMODATIONS FOR A DISABILITY
Ivy Tech Community College seeks to provide effective services and accommodations for otherwise qualified individuals with documented disabilities.
To request special services or academic accommodations, you must:
- Contact Disability Support Services at the telephone or address below (No action can be taken without a specific request from the individual needing the assistance.)
- Complete and return the request form.
- Provide us with professional documentation of your disability, that:
- describes the disability.
- gives significant support for the accommodations requested.
Documentation of disability and requests for accommodations should be received at least one month prior to the start of each academic term.
Additional time is required for some requests.Usual sources for this information include:
- High school education plan and special psychological testing.
- Vocational Rehabilitation evaluation.
- Doctor’s letter or medical records.
