21st Century Scholars Request for Appointment

Please use this form to set up appointments with the 21st Century Scholars Support Office staff.

Our appointment hours are from 9 a.m. - 3 p.m., Monday through Friday. Appointments are made in one (1) hour blocks. Please use this form to select a day of the week and time that will normally be suitable for you. Please schedule your appointment at least two (2) business days in advance. We will make every effort to accommodate your appointment requests and will email or call you to confirm your requested appointment day and time.
