High School or GED Transcript Request
If you are seeking an Associate Degree, Technical Certificate, or applying for Financial Aid, you must request an Official High School Transcript from the high school where you graduated. The document is official if it includes: the school's official seal or signature plus your graduation date. Students who have passed the GED exam must submit an official copy of their passing GED scores.

The Official High School Transcript Request form is available in the Admissions Office, or in PDF form. Please fill out this form (Don't forget your authorizing signature.) and send the completed form to your high school to request an official transcript. You will be responsible for any transcript fees due the high school. Your high school will return the form with the transcript to Ivy Tech.

Remember:

  • Official transcript means that the document MUST have an official raised seal or a written signature from a school official and date of graduation.
  • Current high school students must request an official transcript after graduation.
  • The official graduation date is as declared by the graduating high school.
  • The student is responsible for sending this form to the graduating high school.
  • The high school may charge a fee for processing a transcript request.  Contact the high school regarding any transcript fees and remittal.
