Bio/Demographic InformationStudents needing to change personal information may do so by filling out a Change of Information form. The form can be obtained from the Office of Student Affairs.

Requests to change your name must also be accompanied by one of the following: driver's license or social security card indicating new name.

You may also change your address, telephone number, email address and emergency contact information on through Campus Connect (http://cc.ivytech.edu).
